SWEEPSTAKES OFFICIAL RULES
The following Sweepstakes is intended for viewing in the 48 contiguous United States and the District of Columbia only (excluding HI, AK and Puerto Rico) and shall only be construed and evaluated according to United States law. Do not proceed in this site if you are not a legal resident of and located in the United States (excluding HI AK and Puerto Rico) at the time of entry.
ELIGIBILITY: NO PURCHASE NECESSARY TO WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. Sweepstakes open to legal residents of the 48 contiguous states and the District of Columbia who are 18 years or older at the time of entry. Residents of HI, AK and Puerto Rico and other territories are ineligible. Employees of Medimetrix Group ("Sponsors"), their subsidiaries, affiliates, prize suppliers, and advertising and promotion agencies of Sponsors, their immediate families (spouse, parents, children, siblings and their spouses) and individuals living in the same household as such employees are ineligible. Sweepstakes is subject to all applicable federal, state, local laws and regulations. Void in HI, AK, PR and where prohibited by law.
HOW TO ENTER: Sweepstakes begins 12:01 A.M. Eastern Standard Time (EST) September 1, 2007, and ends 11:59 P.M. EST August 31, 2008 (“Entry Period").
A. Online: To enter online, during the Entry Period go to http://www.spiritofwomen.com/getwhatyouneedoutoflife/winwithspirit.html. Follow the directions to complete the online entry form and click submit. Entrants must complete all data requested on online entry form.
B. By Mail: If you do not wish to enter online, you may still enter the sweepstakes by printing your name, address, city, state and zip code, day and evening phone numbers and e-mail address on a postcard (no envelope please) and mail to: Attn: Kate Siver, Spirit of Women/Get What You Need Spa Escape, 2424 N. Federal Highway, Suite 100, Boca Raton, Florida 33431. Mail-in entries must be postmarked by August 31, 2008.
C. By Registration: Those individuals who attend the Get What You Need Out of Life Events may register to participate in the Sweepstakes. To be eligible, registration forms must be completed by the day of the Get What You Need Out of Life event.
By entering this Sweepstakes, you agree to permit Spirit of Women, participating Spirit of Women hospitals, Get What You Need Out of Life Sponsors and others working on behalf or affiliated with Spirit of Women to use this information for marketing purposes. Limit one entry per person--regardless of entry method. Incomplete entries are void. Entries become the property of Sponsors and will not be returned.
HOW TO WIN: One (1) Grand Prize winner will be selected by random drawing from all eligible entries (online, mail-ins and at Get What You Need Out of Life events) on or about November 30, 2008, by Sponsors whose decisions are final and binding on matters relating to Sweepstakes. Odds of winning depend on the number of eligible entries received. All prizes will be awarded. Limit one prize per person, family or household.
PRIZES: One (1) Grand Prize will be awarded. The Grand Prize is a $500 Spa Escape at a spa selected by the winner.
There will be no substitution of prizes except due to unavailability, in which case, a prize of equal or greater value will be awarded at the sole discretion of the Sponsors. Prizes are not transferable, assignable or redeemable for cash. All applicable taxes are the sole responsibility of the Grand Prize Winner.
NOTIFICATION: Winner will be notified by mail and will receive an Affidavit of Eligibility/Release of Liability. Grand Prize Winner's affidavit must be completed, notarized and returned within fourteen (14) days of receipt. A disqualification will result and an alternate winner will be selected by random if the Affidavit is not returned within the specified time.
By participating in this Sweepstakes and submitting an entry, entrants agree to be bound by these Official Rules and the decision of the judges. By entering Sweepstakes, Winner agrees to waive any and all claims against the Sponsors, their affiliates, employees, agents, and representatives for personal injury or loss that may occur from participation in this Sweepstakes or from the receipt or use of any prize, and grant to Sponsors the right to publicize such winner's name and/or likeness without additional compensation or consideration unless prohibited by law.
GENERAL CONDITIONS: Sponsors, their affiliates, employees, agents, and representatives are not responsible for printing or typographical errors in any sweepstakes-related materials; stolen, late, lost, damaged or illegible entries. Sponsors, their affiliates, employees, agents, and representatives, or any telephone network or Internet service providers are not responsible for incorrect, inaccurate transcription, or loss of entry information for any reason whatsoever. If, in the Sponsors' exclusive judgment, this Sweepstakes becomes compromised in any way, Sponsors reserve the right to withdraw the Sweepstakes offer, cancel any method of entry, void any entries submitted fraudulently, or select winners from all eligible entries received prior to the act that compromised the Sweepstakes. By entering, entrants acknowledge compliance with these Official Rules, including all eligibility requirements.
In the case of multiple entries, only the first entry received will be considered. In the event of dispute, electronic entries will be deemed to have been submitted by the authorized account holder of the e-mail address at the time of the sweepstakes.
